Frameworks that address implementation of clinical pharmacy services will be reviewed in the context of oncology. These frameworks include a pharmaceutical care model similar to the medical care model and another with modules in assessment, bottling, counseling and surveillance.
An initial baseline of what clinical services one already provides can assist in planning other clinical services to add to your setting. Some examples will be shared from one practice environment as well as a recently published checklist for implementation of clinical pharmacy services in an ambulatory environment.
One of the newer clinical services, medication reconciliation, which is also a patient safety initiative, will be discussed in this presentation. Implementing clinical oncology pharmacy services requires commitment for an extended period of time as it is a journey rather than a destination.
